Manthani Temple is sited in Manthani city which is in Karimnagar on the banks of Godavari. It is approximately 72-km from Karimnagar. Gauthameswara is the presiding God of the city and it is also the main temple. The history and source of the temple is not known but the monuments around the temple look like the thousand pillar temple of Hanamkonda in Warangal District. Kakatiya rulers may have reconstructed the old temple at the time of their rule. The sculptures here are worth observing. There is a Shiva Lingam made of black stone with an height of 1 m to 1.25 m placed in the sanctum sanctorum (Garbhagudi) on a beautifully carved platform on which the phallic emblem is installed (Panawatika) as in the 'Somasutra'. Between the Garbhagudi and entrance room there is a small room in which in the inner right corner there is the idol of Lord Vigneswara installed.
